Cebu school enrolment requirements — the full checklist

PSA birth certificate, Form 137, report card, medical, baptismal — what to bring, where to get it, and how long each takes.

The universal checklist (every school, every grade)

  • PSA Birth Certificate — original + 2 photocopies. Get from the PSA outlet at SM City Cebu, Ayala Center Cebu, or via online order (PSA Serbilis). Cost: ₱365. Time: same-day walk-in (1-2 hours), or 3-7 days online.
  • 2x2 ID photos — usually 4 to 6 pieces. White background, recent (within 3 months). Most photo studios in malls charge ₱150 to ₱300.
  • Form 137 (or report card) — from your child\'s previous school. Public school: 3-5 days. Private school: 1-3 days. Some charge a small processing fee (₱100 to ₱300).
  • Good Moral Character certificate — from the previous school (for Grade 7 and up transferees). Same registrar processing as Form 137.
  • Parent ID — government-issued, original + photocopy. Driver\'s license, passport, UMID, or PhilSys (national ID) all work.
  • Proof of address — recent utility bill (Veco, Visayan Water, Globe, Smart) or barangay clearance. Required by some schools for service-area checks.

Extra requirements by grade

Preschool, Kinder, Grade 1 entrants

  • Immunization record from the pediatrician or barangay health center. Some schools accept a parent-signed declaration if the original is lost.
  • Medical exam within 6 months of enrolment, signed by an accredited physician. Cost: ₱500 to ₱1,500 at private clinics.
  • Readiness assessment at most premium preschools — a 30-minute play-based assessment with the school counsellor. Bring your child rested and fed.

Grade 7 entrants (Junior High)

  • Form 138 (Grade 6 report card) — final, with the "promoted" signature.
  • Entrance test fee and scheduled test slot — typically ₱500 to ₱1,200, between October and December the year before.
  • Parent interview availability (most top schools require both parents in the interview).

Grade 11 entrants (Senior High)

  • Form 138 (Grade 10 report card).
  • Junior High completion certificate.
  • PEAC SHS voucher application form (if you\'re using the voucher). The school handles the PEAC submission, you provide identity documents.
  • Career assessment results (some schools, especially STEM strands).

Catholic schools (Grade 1 and up)

  • Baptismal certificate — get from the parish office where your child was baptised. Usually free or ₱50 to ₱100.
  • For older students: Confirmation certificate (Grade 7 entrants who are already confirmed).

Foreign students (international curricula)

  • Passport copy (bio page).
  • Valid student visa or SSP (Special Study Permit).
  • Previous school records in English; if originals are in another language, an official translation.
  • ACR I-Card (Alien Certificate of Registration), if stay is over 60 days.

The "I forgot" rescue list

If a school registrar tells you something is missing, here is what you can usually still do same-day:

  • PSA birth cert — SM City Cebu PSA outlet, walk-in.
  • Barangay clearance — back to your barangay hall.
  • 2x2 photos — any nearby mall photo studio (Robinsons, Ayala, SM).
  • Medical exam — drop-in at a Generika or Watsons clinic; or a barangay health center if free is fine.

Form 137 and baptismal usually need a separate trip. Plan for it.

One practical tip

Te, scan everything as you go. Make a folder on your phone (or Google Drive) called "Anak Enrolment 2026-27". Every time you get a paper, take a clear photo of it. When the registrar asks for an additional copy, you can email it instantly. Saves a trip back to the photocopy machine.
