- PSA birth cert — SM City Cebu PSA outlet, walk-in.
- Barangay clearance — back to your barangay hall.
- 2x2 photos — any nearby mall photo studio (Robinsons, Ayala, SM).
- Medical exam — drop-in at a Generika or Watsons clinic; or a barangay health center if free is fine.
Form 137 and baptismal usually need a separate trip. Plan for it.
One practical tip
Te, scan everything as you go. Make a folder on your phone (or Google Drive) called "Anak Enrolment 2026-27". Every time you get a paper, take a clear photo of it. When the registrar asks for an additional copy, you can email it instantly. Saves a trip back to the photocopy machine.
